Peritoneal Mesothelioma Desi Aunty

Peritoneal mesothelioma is a rare condition characterized by diffuse invasion of peritoneal surfaces. The incidence of pleural mesothelioma and peritoneal universally has increased since 1970. In industrialized countries, its incidence is now estimated 0.5 to 3 cases per million inhabitants in men and 0.2 to 2 cases per million in women

The average age of onset is 50 years (17-76 years) and this disease affects the same frequency of the two sexes. His prognosis is very guarded in the absence of appropriate therapeutic management or simple medical treatment with a median survival estimated at 1 year. For cons, the prognosis is better in women because of less invasive histological forms

Etiologies:

Compared with pleural mesothelioma, there are still doubts about the relationship between exposure to asbestosis and the development of peritoneal mesothelioma although several recent studies appear to establish, especially in humans. Many carcinogens have been identified since the first reported case of pleural mesothelioma in relation to a notably asbestosis. Simian Virus-40 has been implicated and may be a cocarcinogen. Other possible causes such as abdominal radiotherapy, chronic peritonitis, exposure to mica, administration of thorium dioxide were discussed. Roushdy-Hammady et al. also recently reported a genetic susceptibility possible, autosomal dominant, in a region of Turkey (Cappadocian) following exposure to erionite, another carcinogen implicated.

Clinical presentation and diagnostic methods:

The circumstances of discovery and symptoms of this disease are mainly the increase in abdominal girth (due to the presence of ascites or tumor formation) and non-specific abdominal pain. The alteration of the overall condition is rare and found only in case of aggressive histologic types. The appearance of a symptomatic hernia is reported in 10% of cases. This mode of revelation is more common in men. Incidental finding during laparoscopy is more common in women.

Morphological examination of reference for diagnosis, staging and monitoring of peritoneal mesothelioma remain today like all peritoneal carcinomatosis, the scanner thoraco-abdominopelvic. But it can detect the lesions more than 5 mm and it largely underestimates the extension of intraperitoneal disease.
